Stuck Code Reviews No Longer Sit Silently Forever

A review that doesn't finish properly now shows up as something to look at, instead of leaving its pull request waiting with no sign anything went wrong.

Summary

Every pull request a reviewing agent looks at is supposed to end with a clear verdict — approved, or changes requested. If a review run ended without actually recording one, the pull request used to sit waiting indefinitely, with nothing telling you it needed attention. Reviews now either deliver a real verdict or are flagged for you to see, so nothing silently stalls.

What You Gain

Previously, a review could finish its work, leave comments, and still never formally approve or request changes — and because nothing checked for that, the pull request just sat there looking "in progress" with no indication that it was actually stuck. Someone had to happen to notice the comments, realize no verdict had been recorded, and manually nudge things along. For a team relying on agents to review and ship work while they're focused elsewhere, that gap meant work could quietly wait far longer than it should.

Now, if a review ends without a verdict, it's automatically retried once, and if it still doesn't produce one, it's marked as failed and surfaced where you already check on things that need you. You get a clear signal the moment something needs a second look, rather than discovering it only when you happen to check a pull request yourself.

How It Works

  • Every review is held to producing a real verdict — a comment alone no longer counts as a finished review.
  • A review that doesn't produce a verdict is retried automatically once, in case it was a one-off hiccup.
  • If it still doesn't produce one, it's marked failed and shown to you as something that needs a look, rather than being left open-ended.
  • The reviewing instructions were tightened so the agent is clearer about exactly how to submit a verdict, reducing how often this happens in the first place.

Where to Find It

This applies automatically to every project using agent-driven reviews — there's nothing to configure. A flagged review appears wherever you already review items waiting on you.
